The County of Los Angeles Aging and Disabilities Department is seeking qualified organizations to deliver Integrated Care Services under the Adult Protective Services program. The program supports older adults and dependent adults who are at risk of abuse, neglect, or exploitation and who lack sufficient caregiver or family support. Selected providers will coordinate comprehensive care and supportive services designed to help clients maintain independence and safety within their communities. Services include care coordination, access to healthcare and nutrition resources, financial and housing stability assistance, and support with activities of daily living and instrumental activities of daily living. Additional services may include home safety modifications, minor home repairs, hoarding mitigation assistance, mobility devices, respite care, and other related supports. The program emphasizes culturally responsive services and aims to prevent social isolation while promoting dignity, independence, and long-term stability for vulnerable residents across Los Angeles County.
