Community Pathways Services
San Mateo County seeks a qualified contractor to design and operate the Community Pathways Program for the county’s Human Services Agency, Children and Family Services division. The program is an evidence-based, prevention and early-intervention model intended to reduce child abuse and neglect and prevent unnecessary entry into foster care, consistent with the Family First Prevention Services Act. The selected provider will receive self-referrals and referrals from community and county partners, conduct comprehensive needs assessments, and deliver service coordination, case management, and family-centered prevention services. Core responsibilities include determining eligibility for Family First prevention services, delivering and overseeing approved evidence-based practices, ensuring model fidelity, and coordinating with community organizations to address stabilization needs.
