Family Resource Centers Lead Agencies
Alameda County Health is seeking qualified organizations to serve as lead agencies for its Family Resource Centers initiative, delivering school-linked health and wellness services across Oakland, San Leandro, and Hayward. The selected contractors will partner with school districts to provide a continuum of services addressing social determinants of health, including benefits enrollment support, case management, family resource navigation, and health education programming.
