The County of San Mateo Health Department seeks a qualified organization to plan, develop, and lead a Community Health Improvement Plan Community Collaborative. The contractor will establish and manage a multi-organizational structure that elevates community voice and ensures diverse representation in countywide public health planning and implementation. Responsibilities include facilitating collaborative meetings, recruiting and supporting community-based organizations and residents, building governance structures, coordinating stipends, managing logistics, and developing evaluation metrics. The Collaborative will advance priorities identified through the Community Health Improvement Plan, including access to health care services, mental health, and social determinants of health. The contractor will also help prepare community engagement for the next Community Health Needs Assessment. Emphasis is placed on culturally competent engagement, inclusive leadership, bilingual access, and equitable participation. The project supports long-term system improvement, shared decision-making, and sustainable cross-sector collaboration in public health planning across San Mateo County.
