Older Americans Act Request For Proposals
The County of San Mateo Health – Aging and Disability Services Division, serving as the Area Agency on Aging, is soliciting proposals to deliver programs funded under the Older Americans Act and related funding streams. The purpose of this Request for Proposals is to support a coordinated system of home and community-based services for adults age 60 and older, adults with disabilities, and family caregivers throughout San Mateo County. Services include supportive services such as adult day care, transportation, legal assistance, senior center activities, information and assistance, disease prevention and health promotion programs, caregiver support and respite services, Health Insurance Counseling and Advocacy Program services, Medicare Improvements for Patients and Providers Act outreach, and Long-Term Care Ombudsman services. The goal is to enhance independence, protect vulnerable residents, promote equitable access, and prioritize individuals with the greatest economic and social need, including low-income, rural, limited English proficiency, and minority populations. Multiple awards may be made to ensure countywide coverage.
