Include a one-page cover letter signed by the person authorized to negotiate, firm qualifications, at least three references from the previous five years, key staff with availability, on-call approach (response times, escalation, staffing), the service categories (1-6) proposed, a compensation rate schedule as a separate section at the end, confirmation of acceptance of the Standard Consulting Agreement and insurance requirements (exceptions noted in the SOQ), and a list of potential conflicts of interest.
