HOW TO SUBMIT
Proposals are submitted only through the Oregon Health Authority's Smartsheet application form. OregonBuys distributes the solicitation and addenda but is not an acceptable submission method, and proposals sent any other way are rejected. Applicants first complete Document A (eligibility screening and certifications); eligible applicants receive an organization ID by email and a link to Document B (application questions). Documents C (project plan), D (budget and narrative) and F (acknowledgements and compliances) are uploaded as attachments to the Document B form, along with a signed Document E letter of commitment on partner letterhead for each partner. Each applicant may submit only one proposal. To change a submitted proposal, email the Smartsheet technical contact rather than uploading a second version; withdrawals require written notice to that contact. Questions go to the single point of contact, Stephanie Morgan of the Office of Contracts and Procurement, quoting 6312/S-44300-00017937.
